In accordance with National guidelines, all volunteers must complete a volunteer application and have their current photo I.D. checked. A volunteer application properly executed before the start of our Fall season in August is good for both the Fall 2007 season and Spring 2008 season.
A volunteer application can be completed by going to www.eayso.org. After logging in using the username and password previously created, click on "Myeayso". Follow the instructions for completing and printing the application. Volunteers who have previously filled out the application should edit any information that has changed. Print two copies of your application, sign and date them.
Volunteer applications not submitted at our May registration can be handed in on the following dates:
Saturday, August 11, 2007: 8:00 a.m. Park District Park Center
Saturday, August 18, 2007: 8:00 a.m. Ladendorf Park Fieldhouse
Saturday, August 18, 2007: 1:00 pm Ladendorf Park Fieldhouse
Sunday, August 19, 2007: 1:00 pm Ladendorf Park Fieldhouse
Ladendorf Park is located at 2541 Harrison St. (Harrison is an east-west street between Glenview Road and Golf Road. 2541 is 4 blocks west of Shermer)
